Overview

The Greenstone Family Health Team is looking for you. We provide collaborative, primary health care to the Northern Ontario Municipality of Greenstone. This exciting work requires caring, creative, and enthusiastic health care professionals. Presently, we are seeking 2 full time Nurse Practitioners to join our team.

Position Summary

As a vital member of the patient centered team, the Nurse Practitioner will work in a primary care setting and is responsible for providing high quality, holistic health care to the municipality of Greenstone. The Nurse Practitioner shall operate within his/her full scope of practice to perform the duties within the guidelines set out by the College of Nurses of Ontario.

The Nurse Practitioner shall provide primary health care assessment, intervention, follow-up, care planning, health promotion, disease prevention, chronic disease management, and education to patients.
