Planned to be opened in January 2026, Cardinal Creek LTC will welcome and become home to 224 residents from the neighbouring communities as well as the daily destination and inspiration for more than four hundred team and community members.

About the Role:

As the Nurse Practitioner, with accountability to the Medical Director and reporting to the Director of Care, you will role model and enact the organization’s vision of “Creating communities where people of all ages can achieve all they can be” and bring to life our PEOPLE values, which support a workplace where each other’s strengths are celebrated, individual aspirations are recognized and supported, and meaningful connections are made.

Our motto is Making Every Moment Matter and we do this through meaningful interactions with Residents and Team Members - and with all those who frequent our community. The Nurse Practitioner champions this approach and leads by example while being responsible for the development, implementation and evaluation of the residents’ plan of care and quality of life for all. Additionally your leadership and collaboration in research, education and evidence-based practice initiatives will optimize outcomes for the resident, long term care home and the health system.

  • Assists, supports, guides, and provides consultation to the Director of Care, Nurse Managers, nursing staff and interdisciplinary team members regarding challenging clinical situations.
  • Identifies and implements research-based innovations for improving resident care and collaborates with members of the interdisciplinary team and/or community to identify research opportunities and to conduct and/or support research.
  • Any and all other tasks as assigned.

What you will need:

  • Registered Nurse Practitioner in good standing with the College of Nurses of Ontario, or a Registered Nurse Practitioner registered with a regulatory authority in a Canadian jurisdiction, other than Ontario, and holds in that jurisdiction the equivalent of a certificate of registration authorizing independent practice in Ontario as of right exemption Bill 60.

  • Must provide a complete and current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one.
  • As a condition of employment, candidates are required to submit documented results of TB testing (within the last six months or within 14 days upon hire) or a chest X-ray (within the last year), as per Public Health requirements.
