Reporting to his/her designate, Primary Health Care Teams and in collaboration with physicians and health care provider team members, the RN (Nurse Practitioner) will provide Primary Health Care (PHC) services to assigned populations which may include: urban populations, visiting services to rural communities, and/or priority populations (e.g. newcomers, teens, residents in long term care facilities) as determined by community and program needs. As the PHC Team evolves, and based on needs of the population, the position may be required to provide new practices such as nurse-led extended hour clinic services and/or first on-call support. Primary Health Care services include: health assessment, health promotion and education, disease and injury prevention and assessment, treatment interventions; collaboration with, and referral to, other health care providers; participation in quality improvement, patient safety, community development, clinical practice redesign, and evaluation.
