The System Flow Coordinator is responsible for providing single point of contact for clients for transfers, admissions and discharges within the Saskatchewan Heath Authority (SHA). Facilitating physician consults and patient transfers from facility to facility to ensure appropriate patient and system flow across the SHA. Collaborating with other stakeholders to complete repatriation of patients back to the appropriate home hospital. Coordinating the utilization of the electronic assessment system RAI-MDS (hereafter referred to as MDS) as the Screening tool for Continuing Care eligibility and placement and alternate level of care services placement within the SHA. Managing the placement of Continuing Care clients into the appropriate vacancy. Providing an effective program and continuous quality improvement and management practice to ensure efficient utilization of the system in consultation with Directors of assigned Integrated Service Areas. Maintaining and Reviewing the Acute Care Dashboard for Bed management purposes.
