Pensionable Salary Enhancement - Nurses in eligible classifications may be eligible to earn an additional $5.95/hour when working full-time hours within a prescribed six-month reporting period. (Effective April 1, 2025 – March 31st 2027.)

Position Overview

Provides clinical support and assessment services by answering calls 7 days a week. Conducts professional assessments of callers’ needs, offering health-related information, guidance on decision-making regarding the need for professional care, appropriate care level and urgency, and support with identifying resources, referrals, and follow-up as required. Utilizes clinical education, experience, approved written protocols / criteria and the nursing process to deliver safe, evidence-based service. Accurately documents demographic information and call details in the electronic health record. Applies principles of client empowerment and health promotion in all interactions.

Processes home care program nursing referrals from community sources such as physicians, walk-in clinics, outpatient departments, hospitals out-of-of province or country for Manitobans who have received health care in Manitoba; in addition to providing community nurses with assistance in problem solving concerning operational and clinical issues after hours.

Provides nursing clinical support to members of the interdisciplinary team while maintaining professional standards set by the organization and regulating body.

Experience
  • Minimum 2 years’ experience of relevant clinical experience in a community setting preferred.
  • Previous related work experience in Home Care as an asset.
Education (Degree/Diploma/Certificate)
  • Graduate of an approved Registered Nursing education required.
SPECIAL TRAINING
  • Community health nursing or relevant experience preferred.
Additional Wound Care, Chronic Disease Management And Certifications Preferred
Certification/Licensure/Registration
  • Maintain good standing with College of Registered Nurses of Manitoba.
  • Responsible for maintaining proof of registration with the College of Registered Nurses of Manitoba.
  • Valid Manitoba Class 5 Driver’s License required and use of a reliable motor vehicle suitable for all environmental conditions.

  • Knowledge of:
    • College of Registered Nurses of Manitoba Standards of Practice for Registered Nurses.
    • Canadian Nurses Association Code of Ethics for Registered Nurses.
    • Community Health Nurse Standards of Practice and Home Health Competencies.
    • Scope of practice documents as documented in the Registered Nurses Act.
    • Regional/Facility policies, procedures, protocols, and guidelines.
    • Nursing care knowledge related to community nursing.
    • Personal Health Information (PHIA), Protection of Persons in Care Act, Mental Health Act, Workplace Hazardous Material Information System.
    • WHMIS, Principles of Routine Practices and other legislated acts.
    • Roles and responsibilities of members of the health care team.
    • Principles of delegation and assignment.
    • Principles of adult learning and teaching strategies.
  • Excellent communication and advanced triage and assessment skills required. Ability to establish good rapport with callers, while accepting limitations, setting and maintaining professional boundaries. Must display behavior in alignment with the mission, vision and values of the Winnipeg Regional Health Authority and the nursing profession.
  • Demonstrated critical thinking and problem-solving.
  • Ability to prioritize care for an individual or group(s).

Physical Requirements
  • Must meet the physical requirements of the job.

This position requires a current satisfactory Criminal Records Check (including Vulnerable Sector Search), Child Abuse Registry Check and Adult Abuse Registry Check as conditions of employment. The successful candidate will be responsible for any service charges incurred. A security check is considered current if it was obtained no more than six (6) months prior to the start of employment.
