About Hospital in the Home (HITH)

HITH at Sir Charles Gairdner Hospital (SCGH) extends our pioneering Home Link service by providing acute inpatient care in patients’ own homes. The multidisciplinary team delivers the same high‑skill staffing, hospital technologies, equipment, medication and safety standards that patients expect in hospital. Patients are visited daily and may be seen by nurses up to twice daily until discharge.

Key Role Responsibilities

You will provide one‑to‑one care to patients both at our hospital clinic and in their homes, delivering treatments such as intravenous antibiotics, wound management, clinical assessment, and heart failure management. You will work autonomously but have immediate guidance and support throughout your shift.
