Job description
Job Purpose

Addictions and Mental Health – responsible for working within an interdisciplinary team to provide a full range of social work services to clients, their families and communities. Monitoring clients from within a Provincial Correctional Center with serious mental health conditions who have been issued a supervised community care plan, or to initiate a supervised community care plan for clients with serious mental health conditions.

Qualifications
  • University degree in Social Work
  • Must be a registered member in good standing with the New Brunswick Association of Social Workers
  • Able to perform work independently and within a team
  • Experience in Addictions and Mental health would be an asset
  • Abide by the professional standards and regional policy and procedures
  • Ability to attend work on a regular basis.
  • Ability to perform the duties of the position.
  • Good work record.
Key Responsibilities
  • Work with community liaisons to ensure treatment team meets obligations outlined in the supervised community care plans
  • Work with partners to ensure processes are followed with a non-compliant client
  • Communicate with stakeholders to support client care and service delivery
  • Contribute to program guidelines to ensure standardization across regions and to support compliance with provincial legislation
  • Consult and guide treatment team, psychiatrists, clinical staff, psychiatric patient advocate, law enforcement and government departments on supervised community care plans
  • Facilitate meetings of the treatment team and related parties
  • Facilitate education sessions with partners, stakeholders regarding the program, processes and legislation
  • Provide guidance, training and orientation to other social worker staff, students, and other health care professionals
  • Assessing clients and developing and implementing treatment plans or programs
  • Developing and facilitating therapeutic interventions for individuals and groups
  • Coordinating services and advocating for clients
  • Providing emergency/crisis intervention/stabilization servicesParticipating in interdisciplinary care planning
  • Maintaining clinical records and documentation
  • Building relationships with critical stakeholders in government and the community
  • Responding to community needs (including schools and other populations/groups) and participating in capacity building initiatives
