Job summary

Keewatinohk Inniniw Minoayawin Inc. is seeking a Social Worker for the Minoayawin Mobile Outreach Program in Thompson, Manitoba, with travel to surrounding First Nations.

The role supports wellness plans across physical, emotional, mental, and spiritual domains in an Indigenous-centered care setting. You will work with Elders, Knowledge Keepers, clinicians, and peer supports to enhance access to health and social services in northern Manitoba communities.

Qualifications

  • Bachelor of Social Work required.
  • Current registration and membership in good standing with the Manitoba College of Social Workers.

Responsibilities

  • Connect with MMOP participants in an Indigenous-specific environment to provide assessment, service planning, treatment, and service coordination.
  • Conduct psycho-social assessments and develop individualized wellness plans with participants, family, and team.
  • Increase access to physical, emotional, mental, spiritual, and social supports.
  • Provide education and service coordination within a multidisciplinary team using anti-racist approaches.
  • Support harm reduction, primary care, health promotion, and prevention services for MMOP participants and communities.
  • Comply with provincial and federal health privacy legislation and relevant acts.
