Job description

Duties:

Under the general direction of the Director and Manager of Patient Care Services, the Mental Health Worker on FACTT (Flexible Assertive Community Treatment Team) provides client-centered, recovery-oriented mental health services. As part of an interdisciplinary team, you will:

  1. Use best-practice and innovative approaches for assessment, treatment, and psychosocial rehabilitation.
  2. Support clients with substance use, housing, employment, relationships, leisure activities, daily living, and transportation.
  3. Implement effective interventions, such as motivational interviewing for substance use.
  4. Build and maintain relationships with employers, service providers, stakeholders, and families.
  5. Collaborate within the FACT team and with community partners, including hospital inpatient units and emergency services.
  6. Support family and informal caregivers as needed.
  7. Provide crisis assessment and intervention on a 24-hour basis, including rotating on-call shifts.
  8. Ensure continuity of care through communication with clients, families, and other service providers, and secure resources via reports and applications.
  9. Develop individualized Recovery Plans of Care with clients and team members.
  10. Handle assessments, treatment, rehabilitation, and support for assigned clients within the FACT team.
  11. Participate in program design, implementation, and evaluation.
  12. Complete all administrative documentation, including workload measurement and quality improvement forms.
  13. Attend daily team meetings to review clients and organize assignments.
  14. Maintain accurate documentation within the Electronic Health Record (EHR) in accordance with hospital policies and timelines.
  15. Advocate for services and opportunities for FACTT clients within the community.
  16. Familiarize with the organization’s strategic plan, goals, and applicable legislation such as the Mental Health Act.
  17. Stay updated through literature review, workshops, seminars, and networking.
  18. Work in compliance with safety practices, policies, and procedures, and promote a discrimination-free workplace environment.

Qualifications:

  • B.A. in Social or Health Sciences required.
  • Certificate or relevant training/experience in Substance Use, Social Work, Occupational Therapy, Vocational Counselling, Behavioral Therapy, or Peer Support preferred.
  • At least 3 years of experience working with individuals with serious and persistent mental illness.
  • Minimum 1 year experience in community mental health is an asset.
  • Knowledge of Psychosocial Rehabilitation principles and applicable legislation (e.g., MHA, Health Care Consent Act).
  • Excellent communication, organizational, critical thinking, and goal-setting skills.
  • Strong observation, assessment, and crisis intervention skills.
  • Leadership experience in group settings.
  • Comprehensive knowledge of community resources (housing, finance, etc.).
  • Ability to work effectively as part of a team, with skills in consensus decision-making and accountability.
  • Training in Suicide Intervention (ASSIST) and Non-Violent Crisis Intervention is an asset.
  • Valid G level driver’s license and personal vehicle are required.
  • Ability to maintain workload measurement and data entry in EHR.
  • Proficiency in English (A-) and French (A-) in oral and written communication is mandatory.
