Duties:
Under the general direction of the Director and Manager of Patient Care Services, the Mental Health Worker on FACTT (Flexible Assertive Community Treatment Team) provides client-centered, recovery-oriented mental health services. As part of an interdisciplinary team, you will:
- Use best-practice and innovative approaches for assessment, treatment, and psychosocial rehabilitation.
- Support clients with substance use, housing, employment, relationships, leisure activities, daily living, and transportation.
- Implement effective interventions, such as motivational interviewing for substance use.
- Build and maintain relationships with employers, service providers, stakeholders, and families.
- Collaborate within the FACT team and with community partners, including hospital inpatient units and emergency services.
- Support family and informal caregivers as needed.
- Provide crisis assessment and intervention on a 24-hour basis, including rotating on-call shifts.
- Ensure continuity of care through communication with clients, families, and other service providers, and secure resources via reports and applications.
- Develop individualized Recovery Plans of Care with clients and team members.
- Handle assessments, treatment, rehabilitation, and support for assigned clients within the FACT team.
- Participate in program design, implementation, and evaluation.
- Complete all administrative documentation, including workload measurement and quality improvement forms.
- Attend daily team meetings to review clients and organize assignments.
- Maintain accurate documentation within the Electronic Health Record (EHR) in accordance with hospital policies and timelines.
- Advocate for services and opportunities for FACTT clients within the community.
- Familiarize with the organization’s strategic plan, goals, and applicable legislation such as the Mental Health Act.
- Stay updated through literature review, workshops, seminars, and networking.
- Work in compliance with safety practices, policies, and procedures, and promote a discrimination-free workplace environment.
Qualifications:
- B.A. in Social or Health Sciences required.
- Certificate or relevant training/experience in Substance Use, Social Work, Occupational Therapy, Vocational Counselling, Behavioral Therapy, or Peer Support preferred.
- At least 3 years of experience working with individuals with serious and persistent mental illness.
- Minimum 1 year experience in community mental health is an asset.
- Knowledge of Psychosocial Rehabilitation principles and applicable legislation (e.g., MHA, Health Care Consent Act).
- Excellent communication, organizational, critical thinking, and goal-setting skills.
- Strong observation, assessment, and crisis intervention skills.
- Leadership experience in group settings.
- Comprehensive knowledge of community resources (housing, finance, etc.).
- Ability to work effectively as part of a team, with skills in consensus decision-making and accountability.
- Training in Suicide Intervention (ASSIST) and Non-Violent Crisis Intervention is an asset.
- Valid G level driver’s license and personal vehicle are required.
- Ability to maintain workload measurement and data entry in EHR.
- Proficiency in English (A-) and French (A-) in oral and written communication is mandatory.
