Overview

Registered Nurse / Mental Health Nurse

This posting is for a current vacancy with St. Joseph’s Home Care. The program is the Integrated Comprehensive Care Program (ICCP) within Clinical Programs.

Responsibilities
  • Clinical: provide quality nursing care within the scope of practice and in accordance with the professional regulating body; maintain knowledge and compliance with the Professional Standards; follow nursing processes and practices efficiently; ensure maintenance of high-quality nursing skills and apply current knowledge to nursing care; comply with SJHC nursing practices, processes and work instructions; review and follow the plan of nursing care including discharge planning; engage in consultation with clients, families/caregivers, multidisciplinary team members and external stakeholders as required (e.g., HCCSS, ICC Care Coordinators); evaluate and monitor care plans on an ongoing basis; assist other clinical programs if requested; participate in ongoing professional development to obtain more advanced nursing skills; complete self-appraisals per the College of Nurses of Ontario Standards of Practice and SJHC practices; advocate for patients and families; complete comprehensive nursing assessments; conduct virtual care visits as necessary.
  • Operational: complete program, clinical or organizational documentation in a timely and accurate manner; participate in organizational learning opportunities; engage in organizational working groups; effectively utilize organizational tools and software; be willing to learn new technical skills; provide care in a variety of geographical locations and environments; act as coach/mentor/preceptor to new nurses through knowledge sharing and peer support.

Qualifications
  • Bachelor of Science Degree in Nursing from an accredited university — required
  • One of the following – required:
  1. IV medication administration as part of the basic educational curriculum;
  2. IV medication administration certificate;
  3. Medication administration certificate plus an IV medication administration certificate
  • Current and unrestricted RN registration with the College of Nurses of Ontario — required
  • Basic Cardiac Life Support (BCLS) — required
  • Previous experience with specific nursing care (e.g., palliative, diabetic, COPD, peritoneal dialysis, etc.) — preferred
  • Previous experience in Mental Health and/or Mental Health Nursing — preferred
