Qualifications
  • Minimum SSW (Or Other Equivalent Education)
  • Registration with respective College (example: OCSWSSW, CRPO, RSSW Or CNO)
  • Ability to work remotely (If applicable)
  • Knowledge of various Acts (MHA, PHIPPA, Health Care Consent Act)
  • Knowledge of DSM5
  • Valid Driver’s license and insurance
  • Reliable vehicle
  • Successful Vulnerable Sector Check within the last 6 months

Duties and Responsibilities
  • Provide life coaching, counseling, assessment, care planning, and referrals.
  • Support clients in managing emotional, social, and mental health needs.
  • Conduct comprehensive psychosocial assessments and develop individualized care plans.
  • Collaborate with healthcare teams, community agencies, and family members to coordinate services.
  • Maintain accurate documentation while adhering to confidentiality and ethical standards.
Skills and Abilities
  • Exceptional interpersonal skills with the ability to work independently and as part of a multidisciplinary team.
  • Strong critical thinking, problem-solving, and active listening abilities.
  • Able to travel locally to meet with clients in various settings.
  • Must be able to effectively communicate both verbally and in writing in English.
  • Ability to communicate both verbally and in writing in French is an asset.
