Definition of work

Reporting to the Regional Director of Primary Health Care (PHC), Chronic Disease Prevention and Management (CDPM) and Child Health, the successful candidate will be accountable for operational oversight of clinical services and staff, providing leadership in planning, organizing, directing, monitoring and evaluating clinical program activities for the Family care Team (FCT) spoke clinics in the Stephenville/Bay St. George Health Neighbourhood, connecting with the FCT Hub site and well as developing and implementing Family Care Team services in the Burgeo/Ramea Health Neighbourhood. In addition, there is responsibility for leadership of PHC and CDPM services with a focus on Enhanced Virtual Care Integration within Primary Care. The Manager is responsible for leading program management and planning, team building, monitoring clinical efficiency, implementation of initiatives to support improvements in the delivery of PHC, CDPM and virtual services which are consistent with regional and provincial directions; lead a comprehensive approach to planning and service delivery of PHC/CDPM/virtual services in the assigned health neighbourhoods: chair local or zonal committees and Working groups; and lead change management processes to support the expansion of FCT interdisciplinary care, CDPM self-management and screening services, and other clinical services with a focus on promoting collaborative practice across program areas and between health professionals and clients in the provision of PHC to improve access and health outcomes for residents of the entire western zone.

Qualifications

The candidate must possess a Bachelor's degree in a Health related discipline or Social Science. Proof of registration and licensure with good standing as a health professional is required. Applicants must have a minimum of 5 years clinical experience in a related health discipline. Two - three years of progressive leadership experience would be an asset. Candidates will have demonstrated strong interpersonal skills, a collaborative leadership style, organizational, communication and decision-making skills as well as an ability to function in a diverse and changing environment. Preference will be given to applicants with a broad understanding of clinical processes, virtual care, primary health care and change management.

A satisfactory record of work performance and attendance is required for this position.
