Reporting to the Director of Care, the Licensed Practical Nurse (LPN) is responsible for providing planned, patient-oriented, goal-directed nursing care in accordance with provincial standards of care.
Key Responsibilities:
- Work as a member of the inter-professional team, applying a person-centered approach to care.
- Complete and ensure accuracy of residents' clinical assessments, care/service plans, and documentation; communicate changes as needed.
- Implement and evaluate care/service plans, making adjustments as necessary.
- Assist with scheduling and participate in resident care conferences when applicable.
- Coordinate with the Director of Care for resident move-ins, move-outs, and transfers.
- Provide direct resident care, administer medications/treatments, and document in compliance with standards and policies.
- Manage medication orders, safekeeping of drugs and narcotics, and administration in accordance with regulatory requirements.
- Facilitate and provide resident and family education.
- Plan, coordinate, manage, and evaluate resident care plans collaboratively with residents, families, physicians, and the interdisciplinary team.
- Coordinate nursing team assignments to ensure efficient care delivery.
- Respond to changes in resident condition or medical emergencies; provide emergency first aid.
- Teach, delegate, and supervise unregulated care providers.
- Coordinate emergency response in the absence of senior management.
- Address resident and family concerns, ensuring appropriate actions and communication.
- Assist in the coordination and monitoring of Infection Prevention and Control, Health and Safety, and Quality/Safety Programs.
- Uphold trust with vulnerable adults, maintaining ethics, morality, and integrity per the Code of Conduct.
- Perform additional duties as assigned.
Qualifications:
- Current registration as a Licensed Practical Nurse in the applicable Province/State.
- Experience in senior care settings and with unregulated care providers preferred.
- Education and experience in gerontology, rehabilitation, or community/long-term care are assets.
- Strong communication, critical thinking, care coordination, time management, and customer service skills.
- Demonstrated clinical knowledge.
- Current Basic Cardiac Life Support and First Aid certifications.
- Proficiency with Microsoft Office applications.
- Ability to provide a current (within six months) Vulnerable Sector Check, including a Criminal Background Check, or willingness to obtain one.
Wage Leveling Salary: $32.84 - $41.35 per hour, based on qualifications and experience.
