Northern Health is seeking a licensed practical nurse to join the Home Support Team as a Team Lead. You will supervise Community Health Workers, coordinate client care plans, provide training and evaluation of personal care techniques, and advocate for patients and families across homes, schools, and communities.

Under the BC College of Nurses and Midwives standards, you will collaborate with an interprofessional team to ensure safe and ethical care.

In accordance with the established mission, vision, values, and strategic priorities of Northern Health, and under the general supervision of Manager or designate, the Licensed Practical Nurse - Home Support Team Lead practices in accordance with the standards of professional practice and code of ethics as outline by the BC College of Nurses and Midwives. Coordinates Home Support Services by supervising Community Health Workers, overseeing the implementation of client care plans, providing training and evaluation of personal care techniques, provides advocacy services, and also functions as a member of the interprofessional team and applies best practice to provide competent, safe, and ethical care for people and their families in settings such as physician practices, homes, schools, and communities.

Shift Rotation/Hours of work: Days, Nights - 07:00 to 19:00, 19:00 to 07:00, Rotating
