Job summary

AgeCare is seeking an experienced Licensed Practical Nurse (LPN) to join our team in Chestermere, AB. Reporting to the Director of Care, you will provide planned, patient-centered nursing care and medication administration in line with provincial standards.

You will complete assessments, care/service plans, and documentation, while supporting move-ins/outs and resident transfers; collaborate with an inter-professional team and mentor staff in a values-driven culture.

Qualifications

• Practical Nurse Diploma is required and current registration in good standing.
• 3+ years of experience in seniors’ health.
• Current CPR training is required.
• Computer proficiency including Microsoft Office applications.

Responsibilities

• Provide direct resident care and administer medications in accordance with policies.
• Complete and update clinical assessments and care/service plans.
• Document care accurately and communicate changes to the team.
• Assist with care conferences and coordinate move-ins/outs and transfers.
• Partner with the Director of Care to support residents and families.
