Job summary

CarePartners is seeking a Personal Support Manager to coordinate and supervise Community Health Services, ensuring high-quality patient care across the Niagara Region and in-office in St. Catharines.

This full-time role includes on-call evenings and weekends on a rotational basis. You will lead Community Support Workers, develop care plans, ensure compliance with health regulations, promote services, and support workplace safety.

Qualifications

• RN or RPN license with CNO registration.
• 3–5 years of job-related experience.
• Experience in community healthcare and leadership is an asset.
• Proven computer skills (email, word processing, scheduling).
• Valid driver’s license and ability to travel.
• CPR certification.
• Background check required.

Responsibilities

• Provides coordination, supervision, and coaching to the Community Support Workers in the field.
• Develops and maintains an effective Care Plan with respect to Community Health Services, independently or in cooperation with the Ontario Health atHome.
• Keeps abreast of policies outlined in the Regulated Health Professions Act and Community Health Services best practices and monitoring employee performance.
• Promotes and markets Community Health Services and maintains positive public relations.
• Provides active guidance for workplace Health and Safety.
• Performs other duties as required.
