Overview
In accordance with the Vision, Purpose, and Values, and strategic direction of Island Health (Vancouver Island Health Authority), patient and staff safety is a priority and a responsibility shared by everyone; as such, the requirement to continuously improve quality and safety is inherent in all aspects of this position.
Working as an integral part of the Intensive Case Management (ICM) Team and under the direction of the ICM Team Leader, the role is that of a Public Health Nurse working in a multidisciplinary outreach team. The role is focused on engaging the hardest to reach population, who have a high level of needs but are not currently well connected to health related care, in core mental health, substance use and public health services. The ICM Nurse role also includes the provision of community clinics for the purpose of engaging the hardest to reach in HIV/STI/hepatitis testing and treatment. The ICM Nurse also provides support to the Medical Health Officer (MHO) in the control and prevention of communicable disease, as per Bill 23, Public Health Act, 2008.
Responsibilities
- Engage with the hardest to reach population to connect them to core mental health, substance use and public health services.
- Provide community clinics for HIV/STI/hepatitis testing and treatment.
- Support the Medical Health Officer (MHO) in the control and prevention of communicable disease per applicable public health acts and regulations.
Qualifications
- Registration with BC College of Nurses and Midwives as a practicing RN(C) registrant.
- STI Certification with the BC College of Nurses and Midwives.
- Immunization Competency BC.
- Graduation from a recognized nursing program with community or public health component.
- A minimum of 5 years' nursing experience, with at least 2 years public health-related, preferably in reportable communicable disease.
- A minimum of six months of experience supporting mental health clients in a community setting or an equivalent combination of education, training and experience.
- A valid BC driver's license.
- Current CPR certification.
Skills And Abilities
- Knowledge of Psychosocial Rehabilitation (PSR) model; assessment and treatment and case management methodologies; learning theories of mental disorder and intervention strategies; knowledge of relevant mental health and addictions legislation and consumer rights.
- Knowledge of community mental health and addictions programs, services, resources and promotion; knowledge of other health disciplines and their role in client care.
- Comprehensive knowledge of communicable diseases and counseling, testing, and prophylaxis of HIV, viral hepatitis, TB and STI; TB and STI treatment.
- Basic skills in epidemiology; HIV Partner Notification and Pre and Post Test Counseling with BCCDC or equivalent.
- Enhanced knowledge of harm reduction philosophy and cultural safety as applied to practice.
- Basic knowledge of mental health issues, illicit drug use and problematic use of alcohol.
- Nursing clinical skills related to examination and testing for communicable diseases (e.g., HIV, viral hepatitis, STI, TB) including venipuncture, pelvic exams, STI exams, pap tests, TB skin and sputum tests, and interpretation of laboratory results.
- First Aid and CPR; ability to identify drug-induced states and withdrawal symptoms; violence prevention and defusing violent behavior.
- Ability to work effectively and safely in an unstructured environment.
- Excellent communication, public relations, interpersonal, and networking skills with the ability to develop professional relationships of trust with clients and guide them toward safer lifestyle practices and mainstream health care services.
- Ability to monitor and report on health-related issues affecting the target population; ability to educate and present to client, professional and community groups.
- Ability to apply a community development approach when working with client and community groups.
- Computer literacy and data entry.
