Job summary

CBI Home Health in Hamilton seeks a Primary Nurse Clinician & Supervisor to lead integrated care teams in delivering advanced home-based care. You will conduct comprehensive assessments, develop individualized care plans, and coordinate timely interventions to improve outcomes and reduce hospital admissions.

The role emphasizes coaching, training, and promoting patient-centred care, quality improvement, and interdisciplinary collaboration to strengthen community-based practice.

Qualifications

• Registered Nurse in good standing with the College of Nurses of Ontario (CNO).
• 3–5 years supervisory or clinical leadership experience in home and community healthcare.
• Advanced clinical expertise and ability to deliver acute and complex care at home.
• Strong leadership, team development and collaboration skills.
• Excellent organization and time management for competing priorities.
• Proficient in Microsoft Office and scheduling/documentation systems.
• Valid Ontario driver's license and access to a reliable vehicle.
• Strong communication and problem-solving abilities.
• Commitment to patient-centred, quality care and continuous improvement.

Responsibilities

• Lead and supervise Nurses and Unregulated Care Providers delivering home-based care.
• Stabilize complex patients through comprehensive assessments and care planning.
• Coordinate with interdisciplinary teams to ensure seamless transitions and prevent hospital admissions.
• Coach, train, and develop team capabilities in patient-centred care and quality improvement.
• Drive continuity of care through effective communication and documentation.
