Job summary

University Health Network (UHN) seeks an Indigenous Community Health Worker to support Indigenous patients at UHN and in community settings. You will develop comprehensive care plans, provide navigation and advocacy, and coordinate with health and social services.

The role emphasizes health equity and culturally safe care, with duties spanning collaboration with programs, building relationships with Indigenous communities, and ensuring access to ceremonial and traditional supports.

Qualifications

• Bachelor degree in health or social work or equivalent experience.
• Strong knowledge of Indigenous health resources and social determinants of health.

Responsibilities

• Provide direct 1:1 Indigenous patient support and care coordination.
• Advocate for patients and connect them with community services and cultural supports.
• Develop and adjust comprehensive care plans addressing health and social needs.
