Qualifications

• Minimum 2 years’ experience in social work practice, preferably in community health.

Responsibilities

• Conduct comprehensive assessments of clients’ needs.
• Develop and implement individualized care plans.
• Provide ongoing case management and crisis intervention.
• Offer therapeutic interventions and support groups.
• Liaise with community resources and healthcare providers.
• Advocate for client rights to social services.
