Qualifications

• PSW certification and one to two years of experience required.
• Current access to a personal vehicle for business use is necessary.

Responsibilities

• Provide direct hands-on care to patients under a Directing Registered Nurse.
• Document clinical data in patient’s electronic medical record.
• Coordinate additional resources required in the home.

Education

Personal Support Worker (PSW) certification

Current CPR and First Aid certification

Developmental Support Worker (DSW) certification

Knowledge of fundamentals of Palliative Care - Level I
