Job summary

University Health Network in Toronto is seeking a Pharmacist for the Outpatient Pharmacy at Toronto Western Hospital with potential cross-coverage at Toronto General and Princess Margaret. This temporary part-time 12-month contract reports to the Manager and offers a wage range of $48.51 to $60.64 per hour.

The role involves reviewing prescriptions, ensuring safety, reconciling medications, and providing drug information while supervising staff and collaborating with a multidisciplinary team.

Qualifications

• Bachelor's or Doctor of Pharmacy degree required.
• Active registration with the Ontario College of Pharmacists (Part A) required.
• Certificate of competency from the Pharmacy Examination Board of Canada required.
• Certificate of competency from an accredited smoking cessation program and flu vaccination program required.
• Familiarity with dispensing processes for specialty medications and for out-of-province patients.
• Experience working as part of a multi-disciplinary team.
• Excellent verbal and written communication skills.
• Experience handling pharmacy inventory.

Responsibilities

• Review, verify, check and dispense prescribed medication orders for appropriateness.
• Identify, resolve and prevent drug-related problems.
• Monitor drug therapy for effectiveness, toxicity and adherence to guidelines.
• Conduct medication reconciliation.
• Promote safe medication practices on the unit.
• Respond to drug information questions.
• Provide supervision of technician staff.
• Perform other duties as required.
