Job summary

SE Health in Canada is hiring Physiotherapists (PT) to join the Acute Care Transitions Program, delivering client-centered care in home and community settings. You will work within a self-directed, interdisciplinary team to plan joint care, addressing physical, cognitive, social, emotional and spiritual needs.

This hybrid role supports remote work and client visits, offering autonomy, mentorship, and opportunities to advance health outcomes across Canada.

Qualifications

• Recent experience in managing patients in acute/rehab or community-based rehabilitation programs is preferred.
• Proven ability to work autonomously within a self-directed team.
• Excellent interpersonal and organizational skills.

Responsibilities

• Conducting assessments
• Formulating goals and recommendations
• Planning and implementing client-centered, goal-directed treatment programs
• Adapting to hybrid work with remote and client-site visits
• Travel to client locations as required
