SE Health is hiring Occupational Therapists to support home and community care through client-centered plans. Independent contract or employment options are available, with a hybrid work model and travel to client sites. A sign-on bonus may be available, and compensation reflects experience within Canada.

Qualifications

• Recent experience in acute/rehab or community-based rehabilitation preferred.
• Ability to work in a collaborative, self-directed team.
• Hybrid work model with remote and client-site duties.
• Valid driver’s license and ability to travel.

Responsabilités

• Conduct assessments and formulate goals and recommendations.
• Plan and implement client-centered, goal-directed treatment.
• Collaborate with an interdisciplinary rehab team.
• Provide care in home, community, or acute/rehab settings.
• Use community resources to support client goals.
