Job summary

A charitable organization in Ottawa is seeking a full-time Case Manager to support clients transitioning from homelessness to permanent housing. This role requires strong case management skills, and experience with urban populations facing addiction and mental health issues. The ideal candidate will have a Community College degree in Social Work and at least 2 years of relevant experience. Competitive salary range from $23.28 to $29.10 per hour, based on qualifications and experience.

Qualifications

• Minimum 2 years experience in direct client case management.
• Proven experience in liaising with community resource agencies.
• Current certification in First Aid / CPR ASSIST and NVCI preferred.
• Valid G driver’s license and current clean driver abstract.

Responsibilities

• Engage clients in their environment.
• Case management including intake assessment for housing.
• Provide crisis intervention and support to clients.
• Maintain accurate records and documentation.
• Refer clients to community resources as needed.
