Job summary

Main Street Project in Winnipeg is seeking a Housing Case Manager to support unsheltered individuals through a housing-first, harm-reduction lens. You will partner with the interdisciplinary team and community agencies to assess needs, create care plans, and advocate for participants in securing housing, income support, and health services.

The role emphasizes trauma-informed practice, cultural safety, and collaboration with landlords and service providers to reduce barriers and improve outcomes

Qualifications

  • Bachelor’s Degree or frontline experience with unsheltered individuals.
  • Knowledge of case management principles: eligibility, assessment, planning, implementation, evaluation, documentation.
  • Experience creating and maintaining case plans and progress notes.
  • Experience working with Indigenous organizations/communities.
  • Ability to maintain confidentiality and handle sensitive information.

Responsibilities

POSITION: Housing Case Manager

DEPARTMENT: Case Management

REPORT TO: Manager, Case Management

POSITION TYPE: Full-time, 6 months term

LOCATION: 637 Main Street

HOURS OF WORK: Days (07:30 to 16:00)

SHIFT SCHEDULE: Week 1: Sun Mon Tues Wed Thurs Fri Sat – O D D D D D O

JOB SUMMARY

The case management team is a key connection to outside organizations and participants seeking services. They work with individuals who access MSP services to identify, develop, implement, and assess self‑identified goals. Case managers work from a housing‑first, harm‑reduction, and trauma‑informed lens to support people in finding and maintaining housing, connecting them to primary care, income, addiction, and mental health supports, breaking down barriers, and advocating for participants. Relationship building is the key function of this team.

KEY RESPONSIBILITIES
  • Provide ongoing case management support to assess progress and ensure the treatment plan is followed.
  • Develop and coordinate individualized care plans addressing short‑term and long‑term goals in collaboration with participants, interdisciplinary team members, and external partners.
  • Assess an unsheltered person’s needs, identify barriers, and develop strategies that facilitate the participant’s ability to meet identified goals.
  • Create and maintain a professional relationship with community members, including boundary setting.
  • Keep accurate, continuous, legible, and signed records including progress notes, checklists, correspondence, and journals in accordance with established protocols.
  • Work with involuntary participants and participants not capable of consent.
  • Apply ethics of case management in all participant relationships, including confidentiality, self‑determination, informed consent, and power balance.
  • Plan process and intervention with participant.
  • Identify and provide information and resources to participants:
    • Community resources
    • Rent and income assistance
    • Mental health resources
    • Indigenous organizations
  • Establish partnerships with landlords and service agencies, and advocate on behalf of participants.
  • Identify symptoms of suicide ideation and offer resources and support.
  • Create environments of cultural safety for participants.
  • Meet with participants both on‑site and in community.
  • Monitor, evaluate, and communicate progress of participants with the participants themselves, team members, and managers on a regular basis.
  • Maintain accurate and confidential electronic and file records, including statistical data.
  • Conduct crisis intervention as needed.
  • Attend program‑related community, coalition, and committee meetings.
OUTREACH DUTIES
  • Build effective relationships with encampment residents.
  • Coordinate with local agencies to provide services that meet the needs of residents.
  • Contribute to the development of a community that values cultural diversity.
  • Preferred experience with homeless outreach, working with homeless individuals/families, accessing a wide range of services, and familiar with overcoming barriers faced by homeless people.
HOUSING (THE BELL)
  • Provide a supportive and proactive approach to eviction prevention.
  • Create an open and welcoming environment for tenants to share housing concerns and address needs.
  • Review client risks or issues and utilize a team approach to identify an action plan.
  • Experience with palliative and after‑death care.
  • Experience with harm reduction in a supportive living environment.
MMIWG2S
  • Provide comprehensive case management services to families affected by MMIWG, demonstrating sensitivity to cultural nuances and diverse needs of Two‑Spirit individuals.
  • Establish and maintain supportive relationships with families, acting as a liaison between them and relevant law‑enforcement agencies, social services, and other community resources.
  • Facilitate cultural ceremonies, healing circles, or support groups to address the emotional and spiritual needs of affected families and individuals.
  • Collaborate with local Indigenous communities, Elders, and cultural advisors to ensure cultural perspectives are integrated into case management practices.
  • Contribute to the collection of data for research purposes to inform systemic changes and improve support services.
QUALIFICATIONS
  • Bachelor’s Degree in a relevant field, or experience working frontline with people who are unsheltered, lived experience, or equivalent education and experience.
  • Demonstrated knowledge of case management principles and processes – eligibility, assessment, planning, implementation, evaluation, transitioning, documentation, and file management.
  • Demonstrated experience developing and implementing care plans that promote positive health outcomes.
  • Experience creating and maintaining case plans, progress notes, and other necessary documentation.
  • Experience working with Indigenous organization and/or communities.
  • Experience working with culturally diverse populations.
  • Computer proficiency in Microsoft Office and any other database software used to track service delivery.
  • Training in non‑violent crisis intervention, mental health first aid, suicide intervention, and/or trauma‑informed practice is an asset.
KNOWLEDGE, SKILLS AND ABILITIES
  • Strong knowledge of MSP Van Outreach work and relationships with people living unsheltered and in Winnipeg encampments.
  • Ability to apply a harm‑reduction approach and housing‑first principles to day‑to‑day work.
  • An understanding of and ability to work within the frameworks of client‑centred care, self‑determination, strength‑based approach, and trauma‑informed care.
  • Excellent interpersonal skills and the ability to form relationships with community members, including a proven ability to communicate appropriate professional boundaries.
  • Motivational interviewing skills.
  • Excellent verbal and written communication skills.
  • Knowledge of Indigenous History and Colonial History and its impact on the reality of Indigenous people in Winnipeg.
  • Knowledge of TRC Calls to Action, MMIWG Call to Justice, and UNDRIP.
  • Indigenous language skills are considered an asset.
  • Knowledge of community resources available throughout Manitoba.
  • Knowledge of addictions, mental health diagnoses, and co‑occurring disorders.
  • Knowledge and ability to adhere to confidentiality standards and legislation.
  • Ability to manage, assess, and work with challenging behaviours.
  • Ability to work independently, under pressure, in a team or independent setting.
  • Ability to effectively resolve and cope with conflict and/or crisis situations.
  • Strong collaboration skills, both with team members and with external partners.
  • Ability to receive and apply feedback.
  • Ability to model professional behaviours and demonstrate accountability with participants.
ADDITIONAL REQUIREMENTS
  • Satisfactory Criminal Record Check with Clear Vulnerable Sector Search and a Clear Adult Abuse Registry Check (employee is responsible for any service charges incurred).
  • Valid driver’s licence.
  • Current CPR & basic first aid.
CONDITIONS OF EMPLOYMENT
  • Ability to meet the physical demands required for the role, including being on your feet for long periods, lifting up to 50 lbs, and performing cleaning duties (bending, reaching, pushing/pulling, crouching).
  • Exposure to biohazards including, but not limited to, blood, bodily fluids, and syringes.
  • Ability to work with individuals who use substances and/or experience mental health issues.
  • Ability to provide support in high‑intensity incidents including medical emergencies and aggressive individuals.
  • Comfortable working in an environment where Indigenous ceremonies and practices (including smudging) are actively taking place.
  • Ability to conduct participants in community settings such as encampments, hotels, public housing, etc.
  • Comfortable providing transport for community members in a company vehicle.
  • Ability to occasionally flex hours to meet participant needs.
