Qualifications

• Must have completed a minimum of 75 hours of classroom and supervised practical training.
• Willingness to submit to a criminal background check prior to hire.
• Preferred to have one (1) year experience in community health/hospice.

Responsibilities

• Perform personal care needs for patients as assigned.
• Provide patient and family communication within ethical guidelines.
• Report on patient's condition and changes to the Case Manager.
