Job summary

TLC Nursing Home Care Services Limited is seeking a compassionate Support Worker to provide individualized in-home and community support for clients with behavioral and developmental challenges. The role emphasizes independence, meaningful activities, and safety.

You will collaborate with a care team to follow care plans, assist with personal care and meals, arrange transportation, and document changes in client condition. Training in behavior support is preferred.

Responsibilities

• Provide companionship and emotional support.
• Support clients with behavioral and emotional regulation strategies.
• Accompany clients on community outings, appointments and activities.
• Provide transportation when required.
• Follow individualized care plans and safety protocols.
• Communicate with the care team regarding client needs, progress, and concerns.
• Complete required documentation and report any changes in the client’s condition or behavior.

Home Support Worker (Residential Support)

We are looking for a compassionate, reliable, and patient Support Worker to provide individualized support to clients with behavioral and developmental challenges.

This is a rewarding opportunity for someone who enjoys building positive relationships, supporting independence, and helping clients participate in meaningful activities both at home and in the community.

What You’ll Do

As a Support Worker, you will:

• Provide compassionate, person-centred support in the client’s home and community.
• Assist with personal care, including hygiene, dressing, grooming, and mobility.
• Provide medication support as directed and according to the client’s care plan.
• Prepare meals and assist with light household tasks.
• Provide companionship and emotional support.
• Support clients with behavioral and emotional regulation strategies.
• Accompany clients on community outings, recreational activities, appointments, and other activities.
• Support outings that may include interacting with a variety of therapeutic animals.
• Provide transportation when required.
• Follow individualized care plans and safety protocols.
• Communicate with the care team regarding client needs, progress, and concerns.
• Complete required documentation and report any changes in the client’s condition or behavior.

Requirements

• Valid driver’s licence and reliable access to a personal vehicle are required.
• Ability to provide transportation to clients when required.
• Current First Aid/CPR certification, or willingness to obtain.
• Satisfactory criminal record/vulnerable sector check and other required screening.
• Ability to provide medical including TB test and vaccine record.
• Ability to complete all required training.
• Able to work 12 hour shifts (mix of days and nights).
