  • Educate patients and caregivers on exercises, safety precautions, and self-care strategies.
  • Collaborate with an interdisciplinary team to provide holistic, patient-centered care.
  • Maintain accurate and timely documentation of patient progress and treatment plans.
Qualifications:
  • Active Physical Therapist license in Oregon, or the ability to obtain.
  • Master or Doctorate degree in Physical Therapy from a CAPT accredited school.
  • Minimum of 1 year of experience as a Physical Therapist.
  • Home health experience is strongly preferred.
  • Fluent in English.
  • Current BLS, hands‑on CPR certification.
  • Reliable transportation with a valid driver’s license and auto insurance.
  • If education is from a non‑CAPT accredited school, an evaluation by a credentialing agency, such as FCCPT, is required.
