Qualifications

  • Membership in good standing with the College of Nurses of Ontario, Physiotherapists of Ontario, Occupational Therapists of Ontario, or Social Workers.
  • Recent experience in community health or related field.
  • Knowledge of the health care delivery system and community resources.

Responsibilities

  • Assess care needs and determine eligibility for services.
  • Develop individual care and service plans.
  • Link patients with service providers.
  • Monitor care plan delivery.
