Qualifications
- Membership in good standing with the College of Nurses of Ontario, Physiotherapists of Ontario, Occupational Therapists of Ontario, or Social Workers.
- Recent experience in community health or related field.
- Knowledge of the health care delivery system and community resources.
Responsibilities
- Assess care needs and determine eligibility for services.
- Develop individual care and service plans.
- Link patients with service providers.
- Monitor care plan delivery.
