Job summary

Prairie Mountain Health in Boissevain, Manitoba is seeking a Home Care Attendant to provide direct client care and environmental support in home settings, following established standards and procedures.

You will assist clients in maintaining independence and daily living activities, and collaborate with the interdisciplinary team to ensure safe, compassionate care while respecting confidentiality and privacy.

Qualifications

• Health Care Aide Certificate from a recognized Health Care Aide program.
• Other combinations of suitable education and experience may be considered.
• Manitoba Class 5 Driver's License or equivalent with access to a vehicle for Prairie Mountain Health service area.
• Proficiency with Microsoft programs (Outlook, Word, Excel, Access and PowerPoint) and IT skills.
• Demonstrated problem solving and decision-making skills.

Responsibilities

• Demonstrates respect for client independence, dignity, privacy, and lifestyle at all times.
• Supports clients’ independence in activities of daily living, as assessed by the Case Coordinator.
• Demonstrates knowledge of safety and sanitation standards including guidelines for hand washing.
• May be assigned additional tasks as required per PPG-00819, Assignment of Medication and Treatment Tasks to Home Care Attendants.
• Demonstrates ability to effectively utilize electronic programs.
• Performs all communication functions (mail, email, photocopying, faxing, telephone) as required in PHIA and Privacy Act.
• Assists with meal preparation in accordance with the Canada Food Guide or client diet.
• Organizes work according to client’s needs and service requests.
• Reports changes in client status to Supervisor/Case Coordinator as per internal procedures.
• Participates in staff meetings, case conferences, and in-service programs.
• Other duties as assigned.

Position Summary

The Home Care Attendant provides direct client care and environmental support within established standards of care, policies and procedures. The Home Care Attendant functions to meet the needs of clients, assisting them in maintaining optimum health and independence for as long as safely possible in their home environment.

• Demonstrates respect for client independence, dignity, privacy, and lifestyle at all times.
• Supports clients’ independence in activities of daily living, as assessed by the Case Coordinator.
• Demonstrates knowledge of standards for safety and sanitation including guidelines for hand washing. Reports concerns of knowledge and/or skill deficits to the Supervisor.
• With additional education and in accordance with PPG-00819, Assignment of Medication and Treatment Tasks to Home Care Attendants, may be assigned additional tasks as required.
• Demonstrates ability to effectively utilize electronic programs.
• Performs all communication functions (mail, email, photocopying, faxing, telephone) as required in accordance with Personal Health Information Act (PHIA) and Privacy Act.
• Assists with meal preparation in accordance with the “Canada Food Guide” or the client specific diet.
• Organizes work according to client’s needs and the assigned service request.
• Reports any changes in the client status or home environment to the Supervisor and/or Case Coordinator as per PPG-00738, Observation Reporting- Home Care Program and PPG-02154, Observation Reporting- Supportive Housing Program
• Reports incidents immediately to the Supervisor or Case Coordinator and/or the client contact and enters in the Incident Reporting System as required.
• Participates in staff meetings, case conferences, and in-service programs as required. Communicates with co-workers involved to achieve the client-centered goals.
• Other duties as assigned.
