Job summary

Fraser Health Authority is seeking a Community Health Worker 1 to support client independence and assist with housekeeping duties as outlined in the care plan. Travel is required and a personal vehicle with a valid BC driver's license is necessary.

The role involves observing client conditions, maintaining records, and providing companionship and basic care under supervision. It requires a Grade 12 education and FoodSafe certification, with prior related experience preferred.

Qualifications

• Grade 12.
• Class V BC Driver's License and access to personal vehicle with appropriate business insurance coverage.
• FoodSafe.
• Recent, related experience of three months.
• Or an equivalent combination of education, training, and experience.
• Or other Qualifications determined to be reasonable and relevant to the level of work.

Responsibilities

• Observes clients and environments, reporting changes and unsafe conditions to supervisor.
• Participates in staff meetings, quality improvement initiatives, committees and in-services, and maintains up-to-date knowledge.
• Completes and maintains related records and documentation such as communication books and progress reports.
• Performs housekeeping duties such as sweeping, mopping, vacuuming, dusting, dishes, laundry, bed linen changes, garbage.
• Plans, prepares, and serves meals; assists with cleanup after meals.
• Assists with ordering and receiving groceries.
• Supports setup and cleanup of personal care under supervision.
• Promotes safety and infection control; reports hazards.
• Demonstrates methods and provides basic information to clients on housekeeping, shopping, meals and activities.
• Provides short-term respite and companionship in the home.
• Performs other related duties within Home Support Guidelines.

The salary range for this position is CAD $24.05 - $25.82 / hour

Detailed Overview

As part of a multidisciplinary health care team, the Community Health Worker 1 supports client independence and promotes a safe work environment by providing assistance with various housekeeping duties and activities outlined in the care plan. Travel is a requirement of this position. Transportation arrangements must meet the operational requirements of FH in accordance with the service assignment and require the use of a personal vehicle and Valid BC Driver's License.

Responsibilities

• Observes clients and their environments, and reports behavioural, physical, and/or cognitive changes as well as unsafe conditions to supervisor.
• Participates in staff meetings, quality improvement initiatives, committees and in-services, and maintains up-to-date knowledge through involvement in appropriate work-related continuing education, as required by Fraser Health.
• Completes and maintains related records and documentation such as communication books and progress reports.

As per Care Plan

• Performs housekeeping duties such as sweeping and mopping floors, vacuuming, dusting, washing dishes, laundry, bed linen changes and emptying garbage.
• Plans, prepares, and serves meals; provides assistance with clean up after meals.
• Assists with ordering and receiving groceries.
• Supports set up and clean up pre and post personal care performed by Community Health Worker 2 or caregiver, by performing duties such as ensuring towels, soap, etc. are readily available for bathing.
• Promotes a safe and healthy working environment by observing routine precautions and infection control procedures, removing obvious hazards related to safety, reporting faulty equipment, accidents, injuries and near misses, and adhering to rules regarding safety.
• Demonstrates methods and provides basic information to clients in relation to housekeeping, grocery shopping, meal planning and preparation, leisure activities etc.
• Provides short-term respite in the home, by performing duties such as sitting, socializing and engaging with clients in therapeutic activities (Strategies & Actions for Independent Living/SAIL exercises)
• Provides companionship by performing home based leisure activities with the client, such as engaging in conversation, reading with/or to the client, playing board games and/or card games.
• Performs other related duties as assigned within the parameters of the Home Support Guidelines.
