Job summary

CONNECT Communities is seeking a Nurse (Health and Wellness Coach) with mental health and addictions experience in Hamilton, ON. You will collaborate with an integrated coaching team to support Life Redesign Plans for individuals after brain injury or stroke.

You will mentor staff, contribute to learning programs, and help advance quality standards. The role involves working with up to 42 residents in condo-style homes, reporting to the Leader - CONNECT Hamilton.

Qualifications

• Must hold Nursing license in good standing with the College of Nurses of Ontario.
• Experience working with people who have mental health and addictions.
• Ability to mentor staff and contribute to staff development.
• Ability to contribute to Life Redesign Planning for individuals.
• Comfort with a flexibly structured, cross-disciplinary culture.

About this position

Nurse (Health and Wellness Coach) with Mental Health and Addictions Experience

We're looking for an excellent nurse with experience working with people with mental health and addictions. Collaborating as part of an integrated, multi-professional Coaching Resource Team you will contribute to the development and implementation of individualized Life Redesign Plans for the people we support after brain injury and stroke. You will participate in the mentoring and development of staff, contribute to our continuous learning program, and help us in building upon our standards for excellence, and our commitment to continuous quality improvement.

You will report directly to the Leader - CONNECT Hamilton.

• Has experience working with people with mental health and addictions
• Is authentic and can lead with who they are rather than what discipline they are in;
• Can help to uncover what matters most to an individual and contribute towards those priorities;
• Can bring their skills as a nurse (with registration in good standing with the College of Nurses of Ontario) to the table;

What the role looks like. Within your scope of practice:

• Guided by the individual’s priorities and goals, the Health and Wellness Coach contributes to the creation of an integrated, multi-professional Life Redesign Plan by supporting the generation of solutions to address factors impacting the person’s goals and plan for Life Redesign. The Health and Wellness Coach serves as a nurse, coach, mentor and teacher for the people we support, families, and co-workers, informs the development of appropriate support strategies for individuals and staff, providing nursing skills as needed for the individual’s health care needs and lifestyle choices.
• The Health and Wellness Coach contributes to CONNECT’s orientation and continuous learning, and helps build on standards for excellence, and commitment to continuous quality improvement.
• Health and Wellness Coaches work as part of an integrated team with coaches from other disciplines and Life Redesign Coaches (support staff), who support people in their homes. Life Redesign Coaches are people with diverse backgrounds whose collective strengths offer a rich resource for supporting people and their work towards life redesign.
• Lean into your experience working with people facing mental health and addictions challenges, as well as brain injury and stroke.

As Health and Wellness Coach you will support the needs of up to 42 individuals in our condo-style, six-home building in Hamilton, supported by on-site service resource, people resource, decision support and admin coaches. Support is also provided by a global leadership team operating virtually from Lake Country, Langley, Saanich Peninsula and Parksville, BC.

Job Types: Full-time, with an on-call component on rotation
