The South East Grey Community Health Centre is a non‑profit organization committed to providing primary care services and health programs to the residents of Chatsworth, Grey Highlands, Southgate and West Grey. We are building a healthier South East Grey Community through: health promotion, illness prevention & comprehensive, client‑centered primary healthcare. Working closely with the Community Engagement & Health Promotion team, the Health Promoter will play a key role in developing, implementing and evaluating health promotion initiatives aimed at improving the overall health & well‑being of the community. This position involves working closely with community members, stakeholders & other healthcare professionals to identify health needs and create effective strategies for health promotion, health equity, and community development. The position will include working at both the Markdale and Dundalk locations.

Responsibilities
  • Program Development: Design, implement & evaluate health promotion programs tailored to needs of community
  • Community Engagement and Development: Build and maintain relationships with community members, local organizations, and stakeholders to promote health initiatives, provide system navigation to clients with barriers
  • Collaboration: Work with the interprofessional team to integrate health promotion into all aspects of patient care and assess inequities and work with stakeholders to promote solutions to reduce them
  • Education and Outreach: Facilitate and arrange for presentations on various health topics and provide information and referrals to community services such as settlement, housing, employment, social assistance, health, etc.
  • Partnership Development: Develop partnerships with key individuals, communities, and organizations that can impact or influence public health, empowering these collaborations to more effectively promote health
  • Community Assessment: Perform community needs assessments and coordinate, develop, implement, monitor and evaluate programs to respond to those needs
  • Advocacy: Advocate for policies & practices that ensure dignified services and safe practices and promote health equity and improve access to health care services
Qualifications
  • Post‑secondary degree in Health Promotion; or undergraduate studies in a related field & in‑depth experience
  • A deep and abiding commitment to equity, diversity, and inclusion in all its forms, and a cultivated ability to apply an anti‑oppressive lens and framework to community engagement, community development, project management, program design and delivery, and relationship building and repair
  • Experience in program development, implementation, monitoring and evaluation
  • Experience working with diverse populations and equity focused initiatives
  • Experience developing and implementing Black Health strategies is an asset
  • Experience supporting clients with system navigation
  • Proven knowledge of case management processes and procedures, behavior change theories, motivational interviewing, social prescribing, etc.
  • Demonstrated ability to work independently and collaboratively within an inter‑professional team
  • High flexibility with strong interpersonal skills that allow one to work effectively in a diverse environment
  • Excellent critical thinking, time‑management and problem‑solving skills
  • Excellent verbal and written communication and presentation skills in English
  • Working knowledge in a second language that reflects the diversity of the community served by SEGCHC, is an asset
  • Strong computer literacy, including proficiency in the use of Microsoft Office is essential and experience with electronic medical records systems (e.g. Practice Solution) is an asset
  • Valid “G” drivers’ license, access to a reliable vehicle, appropriate proof of personal insurance.
