Second Mile Club of Toronto and PEACH
Established in 1937, The Second Mile Club of Toronto (SMC) is a program of the Kensington Health Centre that has provided community support services in Toronto for nearly 80 years.
We offer safe, recreational spaces with accessible and welcoming services for older adults and adults living with physical disabilities. Across multiple locations, The Second Mile Club provides drop-in Seniors Active Living Centres, Adult Day Programs, Case Management for clients living at home, Home Help, Transportation, and Caregiver Support.
The SMC is dedicated to enhancing the social, intellectual, emotional, and physical well‑being of a multicultural community of older adults and adults living with physical disabilities. We strive to deliver safe and timely care with excellent health outcomes while fostering a respectful and inclusive environment where clients’ and members’ needs come first.
The PEACH Program is a collaboration among Inner City Health Associates, Kensington Health, and Toronto Central Home & Community Care Support Services. PEACH operates as an interdisciplinary, mobile, street- and shelter-based palliative care program serving people experiencing homelessness in Toronto. Through an anti‑oppressive framework, harm reduction, and a trauma‑informed approach, the PEACH Program aims to deliver equity‑informed palliative care as a human right.
The Second Mile Club of Toronto (SMC) is seeking a Health Navigator to provide case management and psychosocial support, assess client needs, and determine eligibility for services. This individual will conduct assessments as required and develop care plans based on clients’ needs and preferences on an ongoing basis.
Salary range: $71,000 to $85,000 CAD per annum
Duration of contract: Until May 27, 2027
Based on experience, plus a competitive benefits package.
