Job summary

Chartwell Master Care LP in Brantford, ON is seeking a Health Navigator to work with the Health & Wellness Manager to coordinate resident assessments, care plans, and wellness services within a welcoming retirement community. You will collaborate with residents, families, physicians, and the care team to ensure personalized, high-quality care.

The role requires active nursing licensure (RPN/LPN or RN), CPR/First Aid certification, and strong clinical and communication skills, with emphasis on

Qualifications

  • Registered Practical Nurse (RPN/LPN) or Registered Nurse (RN) in good standing with the provincial College of Nurses.
  • Current CPR/First Aid certification and medication administration knowledge.
  • Experience in retirement living, long-term care, assisted living, or related healthcare setting.
  • Strong assessment, clinical, organizational, and communication skills.
  • Compassionate, collaborative, resident-focused approach to care.
  • Experience working with electronic health records and Microsoft Office.

Responsibilities

  • Complete resident assessments and develop individualized care and service plans
  • Coordinate resident admissions, care conferences, transfers, and discharges
  • Provide nursing care, medication administration, and resident education as required
  • Partner with physicians, families, and the care team to support changing resident needs
  • Maintain accurate clinical documentation and monitor resident wellness outcomes
  • Support infection prevention, quality improvement, and regulatory compliance initiatives

Are you passionate about helping seniors navigate their health journey with confidence and compassion? As a Health Navigator at Chartwell, you'll work closely with the Health & Wellness Manager to coordinate resident assessments, care plans, and wellness services. Additional Requirements
  • Ability to meet the physical demands of resident care
  • Flexibility to respond to emergencies and participate in Manager-on-Duty or on-call rotations, where required
