Located in the city of New Westminster, Queens Park Care Centre (QPCC) is a warm, community-connected site offering specialized inpatient and outpatient rehabilitation, an adult day program, long‑term care, and the soon‑to‑open Patient Access and Transition Home (PATH) unit.
We’re proud to be a small, friendly facility with deep roots in our community and a strong reliance on our multidisciplinary team. Launching in late 2025, the PATH unit will support patients who are medically stable but not yet ready to return home due to physical, functional, or social recovery needs. This unit bridges the gap between hospital and home, offering compassionate, high‑quality care that empowers patients to regain independence and reconnect with their communities.
We’re building a new interdisciplinary team to bring the PATH unit to life and we’re looking for passionate, creative professionals who want to make a real difference. In accordance with the Mission and Values of Fraser Health, the Health Care Assistant works in an acute department under the direction of a nurse (Registered Nurse (RN), Registered Psychiatric Nurse (RPN), Licensed Practical Nurse (LPN), Nurse Practitioner (NP)), in cooperation with the interprofessional care team, and following established care plans and person‑centered care. Provides personal care to patients such as assisting with toileting, bathing, skin and hair care, oral hygiene, grooming, dressing, changing beds, patient positioning and mobilization. Documents on patient charts in accordance with established procedures, observes and reports patient progress, assists patients with meals, transports patients, and performs related general duties.
Grade 10 plus graduation from a recognized Health Care Assistant program or an equivalent combination of education, training and experience. Current Food Safe Level One, as required.
