Overview

The Health Care Aide provides direct client care and environmental support within established standards of care, policies and procedures. The Health Care Aide functions in meeting the special and changing needs of clients and assisting them in reaching and maintaining optimum health and independence within the limits of their abilities under the direction of the team leader.

Responsibilities
  • Assist clients in activities of daily living, striving to maintain self-respect, dignity, and independence by encouraging clients to care for themselves within their abilities, and integrate the resident Bill of Rights (Long Term Care) into care.
  • Follow individualized care plans, provide feedback to improve care, and communicate/report client changes to the team leader.
  • Document care according to policy and update client health records or flow sheets as per policies and procedures.
  • Provide or assist with personal hygiene, grooming, skin care, oral care, dressing, and care of personal belongings.
  • Assist with elimination, including bathroom assistance, bedpan/urinal use, continence care, and perineal care as per care plan.
  • Support mobility and provide nutritional support, including monitoring and encouraging intake as per care plan.
  • Assist with ostomy care, monitor for skin breakdown, and notify the team leader of any concerns.
  • Record observations, measurements, intake and output, and other required data; maintain applicable flow sheets.
  • Assist with application and care of compression stockings, tensors, braces, and splints as directed.
  • Ensure safety measures (side rails, seat belts, wheelchair pedals) and promptly respond to call lights; conduct regular observation rounds as per care plan.
  • Demonstrate appropriate use and monitoring of restraints per policies and procedures where applicable.
  • Assist with eating to promote dignity and safe feeding techniques according to the care plan.
  • Maintain a safe environment by distributing/storing equipment and maintaining cleanliness.
  • Assist with vital signs, oxygen therapy as directed, and documentation of results to the team leader.
  • Assist with procedures for specimen collection, transport, and labeling per policy.
  • Apply creams, ointments, and/or oils safely according to prescribed orders and care plan.
  • Provide validation, orientation, privacy, and confidentiality of client information.
  • Assist in admissions, transfers, and discharges, including body care after death and handling of client valuables.
  • Promote hand hygiene and infection control practices and use of PPE as appropriate.
  • Maintain a clean, stocked environment; report accidents and breakages immediately.
  • Participate in activity programs; organize and facilitate leisure activities for individuals or groups, including evenings and weekends.
  • Demonstrate safe operation of medical equipment applicable to the client service area.
  • Other duties as assigned.
