Job summary

St. Joseph’s Home Care is seeking an Occupational Therapist to contribute to our Integrated Comprehensive Care Program in a community-based setting. You will assess, plan, and implement personalized rehabilitation to help clients regain independence, working closely with clients, families, and a multidisciplinary team.

The role emphasizes collaboration, education, and safe care delivery in clients’ homes, with supervision of OTA/PTA staff and ongoing professional development.

Qualifications

• Bachelor’s or Master’s Degree in Occupational Therapy and registration with COTO.
• Current BCLS/HCP certification.
• 1–3 years of clinical experience (required).
• Experience with complex medical conditions and mobility support (required).
• Familiarity with home and community care and EMR systems.

Responsibilities

• Conduct holistic assessments of cognitive, physical, perceptual, and emotional function.
• Develop and implement individualized treatment plans.
• Educate clients and caregivers on therapy goals, exercises, and adaptive equipment.
• Collaborate with interdisciplinary teams for discharge planning and care coordination.
• Supervise OTA/PTA staff and mentor students as needed.
• Support clients with complex medical conditions in their homes.
