Job description

Reporting to the Director of Community Wellness, the Family Wellbeing Coordinator is responsible for coordinating the Family Wellbeing programs and services. This program will help reduce violence in families, reduce the need to bring Indigenous children and youth into the child welfare system and youth justice systems, and improve the overall health and wellbeing of the communities.

Role Responsibilities
  • Participating in individual client service plan development and follow-up meetings.
  • Assisting with the development of initial assessments and plans of services to outline family backgrounds, strengths, concerns and recommendations.
  • Providing intensive, hands-on assistance and education to families to successfully achieve their goals.
  • Interviewing clients to assess their situations to determine the services required to meet their needs.
  • Preparing monthly treatment plans, progress notes, contact notes and termination reports for family files.
  • Mentoring individuals and their families on topics such as mental health, marital matters, unemployment, substance use, physical abuse, rehabilitation, childcare and medical care.
  • Identifying support services, such as childcare, homemaker services, prenatal care, substance abuse treatment, job training, counselling, etc.
  • Working with Elders and Healers in providing advice and support to families.
  • Developing and monitoring a budget and keeping accurate and up to date reports.
