Job summary

CenterWell Home Health seeks a compassionate Home Health Aide to support patients at home under the supervision of a registered nurse. You will provide direct patient care, assist with rehabilitation goals and daily activities to improve independence and quality of life.

Required are STNA/CNA qualifications, a valid driver's license, reliable transportation, and compliance with HHA/CNA certification standards. Teamwork and patient safety are emphasized within Humana's network.

Qualifications

• Complete Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months
• Must meet applicable state certification requirements
• A valid driver's license, auto insurance, and reliable transportation are required
• Must be in good standing on the HHA Registry (if applicable) and have completed HHA/CNA course to work for a Medicare certified agency.
• Minimum one year experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant

Responsibilities

• Provide direct patient care under RN supervision and according to the Plan of Care.
• Assist with self-administered medications, verify doses, and observe administration.
• Take and report vital signs (temperature, pulse, BP) and abnormalities.
• Assist with bathing, grooming, hygiene, and other activities to maintain comfort.
• Support feeding and monitor nutrition, help with meals and self-feeding as needed.
• Assist with toileting and ostomy care as required by care plan.
