A community support organization is seeking a Part-Time Support Worker to assist individuals with disabilities. The role involves delivering programs and supports, monitoring personal outcomes, and providing emotional support. Candidates should have a high-school diploma and related work experience, as well as knowledge of health-care procedures. This position requires a commitment to enhancing the community living capabilities of individuals supported.

Contributes to the delivery of supports and programs consistent with life plans to meet the needs of individuals in order to enhance their capacity to function in all aspects of community living.
