Job summary

A leading community support organization in Toronto is looking for a Part-Time Support Worker to enhance the capacity of individuals in community living. Qualifications

Responsibilities

  • Deliver supports and programs according to life plans.
  • Assess day-to-day needs and monitor outcomes.
  • Provide personal and physical care including medication administration.
  • Liaise with families regarding individuals' needs.
  • Support life skills training and provide emotional support.

Qualifications
  • A high‑school diploma, 6 months of related work experience and knowledge of health‑care procedures and mechanical equipment – wheelchairs, lifts, G‑tubes and other specialized equipment is required. Post‑secondary education with a focus on Social Services or Developmental Services may be considered as an equivalent with no related working experience required. Successful completion of internal legislated and mandated training is required. A valid driver’s licence is an asset.
Job Summary

Contributes to the delivery of supports and programs consistent with life plans to meet the needs of individuals in order to enhance their capacity to function in all aspects of community living.

Responsibilities
  • Carrying out regularly assigned or prescribed duties related to the provision of services and supports.
  • Supporting assessment of day‑to‑day needs of individuals supported and in the implementation of life plans and monitoring achievement of personal outcomes.
  • Providing appropriate personal and physical care including medication administration and crisis response.
  • Liaising with friends and family as required to address needs and sharing information on behalf of individuals supported.
  • Supporting life skills training and providing emotional support to individuals on a one‑on‑one or group basis.
