As a leader in the field, Community Living Toronto offers supports to over 6,000 individuals within an intellectual disability as well as support to their families, including residential and day support, assistance with employment, community support, early childhood services and respite.

Qualifications
  • A high‑school diploma, 6 months of related work experience and knowledge of health‑care procedures and mechanical equipment – wheelchairs, lifts, G‑tubes and other specialized equipment is required. Post‑secondary education with a focus on Social Services or Developmental Services may be considered as an equivalent with no related working experience required. Successful completion of internal legislated and mandated training is required. Valid driver’s licence is an asset.
Job Summary

Contributes to the delivery of supports and programs consistent with life plans to meet the needs of individuals in order to enhance their capacity to function in all aspects of community living.

Responsibilities
  • Carrying out regularly assigned or prescribed duties related to the provision of services and supports.
  • Supporting assessment of day‑to‑day needs of individuals supported and in the implementation of life plans and monitoring achievement of personal outcomes.
  • Providing appropriate personal and physical care including medication administration and crisis response.
  • Liaising with friends and family as required to address needs and share information on behalf of individuals supported.
  • Supporting life skills training and providing emotional support to individuals on a one‑on‑one or group basis.
