Job summary

A leading support services organization in Toronto is seeking a Support Worker 2 to identify and meet the needs of individuals supported. Responsibilities include case management, providing personal care, and delivering life skills training. The ideal candidate will have a post-secondary education in Social Services or Developmental Services and a commitment to enhancing community living for individuals with intellectual disabilities.

Qualifications

  • Successful completion of required internal training is necessary.
  • Valid driver’s license is an asset.

Responsibilities

  • Identify needs of individuals and deliver supports consistent with life plans.
  • Provide case management and personal care including medication administration.
  • Devising and providing life skills training and emotional support on a one-on-one basis.
  • Maintain documentation, monitoring finances and assisting with locating housing.
  • Provide direction to part-time staff as required.

HOURS OF WORK:

37.5 hours per week

QUALIFICATIONS:
  • Post‑secondary education with a focus on Social Services or Developmental Services is required and knowledge of health‑care procedures and mechanical equipment—wheelchairs, lifts, G‑tubes and other specialized equipment is required.
  • Post‑secondary education in a non‑related field will require an additional minimum of 1 year of related work experience.
  • Successful completion of internal legislated and mandated training required.
  • Valid driver’s license is an asset.
JOB SUMMARY:

Identifies the needs of individuals supported and delivers supports and programs consistent with life plans in order to enhance their capacity to function in all aspects of community living.

RESPONSIBILITIES:
  • Under the direction of a program supervisor, this position is responsible for: Identifying program needs of individuals supported, participating in the development and implementation of life plans, and assisting in monitoring achievement of personal outcomes.
  • Providing case management to individuals on caseload, providing appropriate personal and physical care including medication administration, crisis response, advocating on behalf of individuals supported and liaising with family, medical and other professionals and service providers to address needs.
  • Devising and providing life skills training and emotional support to individuals supported on a one‑on‑one or group basis and referring to professionals when necessary.
  • Maintaining documentation, monitoring finances, ensuring an appropriate standard of living and assisting with locating housing for individuals supported.
  • Providing direction to part‑time staff as required.
