Job summary

Community Living Toronto is seeking a Temporary Family Support Coordinator to provide goal-oriented support to individuals with developmental disabilities and their families. This role involves advocacy, partnership, and resource access for families, alongside developing personalized plans. With a commitment to inclusion, the coordinator will facilitate access to necessary services and community resources, contributing significantly to the welfare of individuals living at home or in facilities.

Qualifications

  • Minimum of 3-5 years of related experience working with children and/or adults with developmental disabilities.
  • 1-2 years working with families is also required.
  • Training and/or certification in CPI (Non-Violent Crisis Intervention) is an asset.

Responsibilities

  • Provide goal-oriented supports primarily to individuals with developmental disabilities living at home with their families.
  • Identify needs and goals, and advocate for individuals' access to resources.
  • Collaborate with community partners to provide comprehensive options.

Qualifications
  • College diploma or Bachelor’s degree in human services or a related discipline.
  • Training and/or certification in CPI (Non-Violent Crisis Intervention) is an asset.
  • Additional assets include Conflict Management, Leadership, Disability Studies, Applied Behavioural Analysis.
  • Minimum of 3-5 years of related experience working with children and/or adults with developmental disabilities, plus 1-2 years working with families.
  • Experience in community program delivery and knowledge of community-based programs.
  • Knowledge of provincial and community support services, community resources, and services for adults with developmental disabilities.
  • Proficiency with MS Office (Excel, Word, Outlook) and other relevant software.
Responsibilities

The Family Support Coordinator will provide goal-oriented supports primarily to individuals with developmental disabilities living at home with their families, and may also support individuals living independently or in facilities. The role involves partnership with individuals and families, promoting inclusion, guiding access to resources, and advocating for their needs.

  • Meet with individuals and families, develop Person-Directed Plans, and identify needs and goals.
  • Assist with accessing community supports, funding, and medical services.
  • Help individuals clarify strengths and needs, make choices, and connect with resources.
  • Advocate and liaise with community agencies, health professionals, and legal entities.
  • Identify support gaps and collaborate with community partners to provide comprehensive options.
  • Participate in committees to facilitate communication and collaboration.
  • Adhere to policies and demonstrate compassion and respect in all interactions.

This is a temporary position for up to one year, with a work schedule of 37.5 hours per week.
