A community support organization in Aurora is seeking a Mental Health Case Manager for a dual diagnosis role. The position offers 21 hours per week on a contract basis until May 2027, with potential for extension. Candidates should have a Bachelor’s degree in Social Work or a related field, along with relevant experience in mental health and developmental services. The role emphasizes advocacy and case management for individuals facing significant mental health challenges.

One .6 FTE Contract to May 2027 (21 hrs/week) (with the possibility of extension or becoming permanent)
Dual Diagnosis Case Manager - The Mental Health Dual Diagnosis Case Manager is responsible for the provision of comprehensive case management and advocacy services to individuals age sixteen and over who are experiencing significant difficulties in daily functioning as a result of serious mental illness and developmental disability. The Mental Health Dual Diagnosis Case Manager collaborates with consumers, community partners, and the general public in response to individually identified goals, with a focus on increasing the quality of life for individuals served. In addition, the Mental Health Dual Diagnosis Case Manager participates in the development of community resources and creative strategies to address service system issues and public education and awareness.
Police clearance that meets agency standards is required. A valid G Driver’s License, access to a reliable vehicle and adequate insurance coverage for business responsibilities, OR access to reliable transportation is required.
