Mental Health Case Manager- Dual Diagnosis (DDX)

One .6 FTE Contract to May 2027 (21 hrs/week) (with the possibility of extension or becoming permanent)



  • Location: Aurora, Ontario L4G3S9 (must be within a commutable daily distance from the office and/or service area)

  • Status: Contract, Part-Time

  • Starting Hourly Rate: $32.13/Hour

  • Office Hours: Monday - Friday - 8:30 am - 4:30 pm, 21 Hours per week

  • Office Expectations: Work in the office to provide team coverage, minimum once per month in addition to in-office meetings, trainings, and supervisions

Job Description

Dual Diagnosis Case Manager - The Mental Health Dual Diagnosis Case Manager is responsible for the provision of comprehensive case management and advocacy services to individuals age sixteen and over who are experiencing significant difficulties in daily functioning as a result of serious mental illness and developmental disability. The Mental Health Dual Diagnosis Case Manager collaborates with consumers, community partners, and the general public in response to individually identified goals, with a focus on increasing the quality of life for individuals served. In addition, the Mental Health Dual Diagnosis Case Manager participates in the development of community resources and creative strategies to address service system issues and public education and awareness.


Position Requirements


  • Bachelor’s degree in Social Work or related discipline (i.e. Health Sciences)

  • Experience: Minimum three years related experience; Cross sectorial systems knowledge for Mental Health and Developmental Services; Knowledge and practice of psychosocial rehabilitation approach and advanced clinical and assessment skills; Counselling, crisis prevention and intervention experience; Experience working with individuals having serious mental illness and a developmental disability; Experience in working with a multi-disciplinary team; Meet clients face-to-face - in their homes, in our offices, and in the community

  • Second language is considered an asset

  • An understanding of cultural diversity is considered an asset


Police clearance that meets agency standards is required. A valid G Driver’s License, access to a reliable vehicle and adequate insurance coverage for business responsibilities, OR access to reliable transportation is required.
