Detailed Overview

The Coordinator, Planning and Systems Development, working closely with designated areas, clinical leadership and management teams, as well as internal and external stakeholders, is responsible for leading, planning, designing, implementing, maintaining and evaluating assigned initiatives that will facilitate system-wide process improvement.

Responsibilities

1. Coordinates the evidence-based planning and design of the assigned initiatives.
2. Determines, develops and evaluates processes, guidelines and pathways for initiatives related to the assigned area(s) that ensure high quality patient care, in collaboration with clinical leadership, medical/physician leadership (as required) and other clinical areas as required.
3. Leads the implementation, monitoring and evaluation ofinitiative activities, ensuring adherence to systems, practices, policies and timelines.
4. Monitors and provides input into budget and resource allocation for assigned initiatives, ensuring their efficient utilization within the defined objectives, plans and budgets.
5. Leads teams to achieve deliverables and meets with key internal and external stakeholders to ensure effective communication at all critical stages of initiatives.
6. Develops and maintains appropriatedocumentation including action and decision logs. Prepares reports, graphs, tables, briefing notes, presentations, issue papers andmakes recommendations based on the data.
7. Liaises with organizations, agencies and the communities to identify and documentdeliverables including issue discussion and resolution, in support of efficient and effective execution of the initiative.
8. Liaises with appropriate stakeholders to ensure that the coordination of information is maintained; prepares reports and planning documents.
9. Monitorsprogress and takes corrective action to ensure thatinitiative reporting requirements are met.
10. Provides education about projects to individuals, groups, organizations and communities as it pertains to successfully completingdeliverables.
11. Works with Communications to develop and implementcommunication strategies among stakeholders.
12. Assists clinical/physician/designated leadership as appropriate and medicine with integrating and coordinating resource planning, including budgeting, human resources, quality improvement, program standards and guidelines, and patient flow and access.
13. Leads in the development of systems and reports that compile information from multiple sources; researches and analyses issues and trends in order to identify data/information required for monitoring budgets, quality and key performance indicators.
14. Partners with Directors, Clinical Nurse Specialists, Managers, and Physicians, and external agencies/contacts in designing and implementing change processes and assists in the re-engineering process required for transition from existing service models to new models.

Qualifications

Education and Experience

Bachelor's degree in a health related field. Five (5) years recent, related health care experience including two (2) years experience in a coordinating/project management and systems development role within a complex health system, or an equivalent combination of education, training or experience.

• Extensive knowledge of the health care system as it pertains to care and service delivery and the interdependence with other clinical programs/services.

• Ability to lead and manage responsively in an environment of constant change and redefinition, including the ability to re-direct and mobilize teams accordingly.
• Ability to facilitate discussion and negotiate with various stakeholders.
• Working knowledge of applicable regulations, legislation and collective agreements.
• Ability to operate a standard PC and effectively utilize a variety of software applications to support the initiative management activities.
• Physical ability to carry out the duties of the position.
