Socket.dev in Peel Region is seeking a Coordinator, Integrated Care Services to support seniors and caregivers through an integrated model of care. You will coordinate client enrollment, scheduling, and case management for the Integrated Care Centre, working with clinical and interdisciplinary teams to plan, document, and deliver person‑centred services.
This 6‑month contract is onsite at Peel Manor, Brampton. 35 hours per week with some evenings and weekends.

The Seniors Services Development Division is committed to delivering person‑centred care through the Butterfly Philosophy of Care, promoting emotional well‑being and enhancing quality of life for seniors and their caregivers.
While you will not be providing direct resident care, you will play an important role in supporting clients, caregivers, and interdisciplinary teams by coordinating services, facilitating access to resources, and helping older adults remain engaged, connected, and supported in the community.
