CarePartners is seeking compassionate Community Support Workers (CSWs) to join the HISH program in the Haliburton region. You will assist with daily living, meals, and home safety while collaborating with an interdisciplinary team to ensure wrap-around, patient-centered care.

Job description

CarePartners has embarked on a new program in partnership with the Ontario Health at Home (OH@Home) Central East (Durham Region) and we are looking for Community Support Workers (CSWs) to work within the High Intensity Supports at Home (HISH) program. These skilled CSWs will work with patients and their families to support care while patients transition through the healthcare system. The HISH program focuses in Wrap-Around Care so that all team members (including Nursing Therapy, Personal Support, Social Work and Community Support Services, are working cohesively to meet the needs of the patient. These PSWs will receive additional training and certification in Gentle Persuasive Approach (GPA).

In this exciting staffing model, Guaranteed HISH CSWs will have guaranteed & consistent hours of work, week over week and have the opportunity to be a respected member of a larger healthcare team striving to provide high quality patient care and positive transition experience for both the patient and their caregivers.

The hours of work will be 7, 12-hour shifts within a 2-week rotation inclusive of weekends:

Two-week Rotation

Mon/Tues/Sat/Sun

0800 - 2000 hrs

Wed/Thurs/Fri

0800 - 2000 hrs

This is an equivalent of seventy-seven (77) hours per bi-weekly pay period!

What The Role Involves
  • Contributing as a valued member of the interdisciplinary care team
  • Assisting with daily living activities, such as personal care (rise and retire), assistance with household tasks, respite care, medication assistance, special functions
  • Promoting a safe environment by demonstrating proper body mechanics, safe transfer techniques, and home safety
  • Providing cognitive, social and/or emotional support to the patient
  • Preparing nutritious, varied and economical meals and snacks, including special diets, as determined by the care plan, using proper food preparation, storage, and hygiene practices
  • Providing patient status reports and participate in patient/family care planning meetings
What You Bring

  • Experience working with patients with significant and complex care needs such as dementia and mental health
  • A valid driver’s license and vehicle for which you are insured
  • Community Support Worker ( Personal Support Worker, Internationally Trained Nurse, Health Care Aid, Developmental Support Worker, Home Support Worker, or completion of first year Nursing program with preference given to those with a PSW certificate and/or equivalency
  • To protect our patients, we require current vulnerable sector check screening, and a clear background check
